- Enrollment Change for VA Educational Benefits (Complete when declared program or class schedule change occurs including adding, dropping, or withdrawing courses.)
- Request to Certify Enrollment Form (VA eligible student must complete form each semester after student has registered for courses.)
- VA Joint Service Transcript Authoritzation Request to Certify Enrollment Form (Complete if student authorizes SoLAcc to request his/her military transcript on student's behalf.)
