Disability Services
The Office of Disability Services at SoLAcc serves as an advocate for students with documented disabilities to help them make a smooth transition to college and ensure equal access to the programs, services, activities, and facilities offered by the college.
Various accommodations and support services are in place to assist students according to their documented needs. Students with disabilities are encouraged to apply for Disability Services and request accommodations prior to the beginning of the semester.
A student is eligible to apply for services and accommodations through the SoLAcc Office of Disability Services if they:
- have a disability or is regarded as having a disability.
- have enrolled at SoLAcc and is currently registered for classes in LoLA.
- have self-identified to the Office of Disability Services and requested accommodations.
- have submitted the required documentation from a licensed professional regarding the disability to the Office of Disability Services.
- have attended the required Intake Meeting and completed an Application Packet.
Steps to apply for Disability Services and request academic accommodations:
- Contact the Office of Disability Services to identify and request accommodations. (disability@solacc.edu or call 337-521-6656)
- Submit documentation as required by the college from a licensed professional to: disability@solacc.edu. *Please refer to the Documentation Guidelines & Criteria in the next tab for the specific documentation required by the college to apply for accommodations.
- Schedule and attend a required, in-person intake meeting with Disability Services. A booking link will be sent to eligible students after review and approval of documentation.
- Complete and submit the APPLICATION PACKET provided by Disability Services.
- The Manager of Accommodations & Disability Resources will review and evaluate the documentation, intake information, and program/course requirements to determine individualized, reasonable accommodations on a case-by-case basis. Additional documentation may be required to determine eligibility and approve accommodations.
Documentation Guidelines and Criteria
Documentation from a licensed professional who is qualified to diagnose and treat the disability is required to apply for accommodations and MUST meet the following criteria:
- Documentation must be a letter or report typed/printed on official letterhead. SoLAcc does not provide templates or forms for practitioners to fill out.
- Documentation must be submitted/emailed to the DS Office by the student.
- Documentation should not be emailed to DS from a clinic/provider/parent for the student.
- Handwritten notes on prescription pads, visit summaries, patient histories, or progress reports are not accepted as documentation.
- Documentation MUST be current (within the last 3 years).
- IEPs and 504 Plans are K-12 plans ONLY. They do not transfer to college and will NOT be a determining factor regarding a student’s eligibility and approval for college accommodations.
DOCUMENTATION MUST INCLUDE THE FOLLOWING INFORMATION TO APPLY FOR SERVICES AND ACCOMMODATIONS:
- The student’s primary medical or mental health diagnosis with DSM-IV-TR code and date of diagnosis.
- Functional limitations, level/severity of impairment, and expected impact of disability on academic performance.
- A clear description of how the disability is impacting the student in an educational environment.
- Suggested accommodations based upon diagnosed condition and functional limitations.
- The provider’s signature and credentials (Name, Title, License or Certification #).
Returning students who are registered with Disability Services have to complete a Semester Renewal Form EACH SEMESTER to continue receiving services and accommodations.
Renewal Forms should be completed at least 2 weeks before the semester begins. Students must be registered in LoLA and use their official school email to complete and submit a Renewal Form.

Student Responsibilities
Students play an active role in the accommodations process by:
- Self-identifying to the Office of Disability Services and requesting accommodations.
- Submitting current documentation from a licensed professional that verifies their disability and recommends accommodations.
- Renewing accommodations each semester to ensure ongoing access to services.
- Communicating with faculty by providing their official accommodation letter.
- Utilizing approved accommodations as needed.
- Note: Provider recommendations are not guaranteed. Course and program requirements may determine the parameters for qualifying reasonable accommodations.
